I’ve been trying shoes like made since the NB Leadville is discontinued. It was not perfect for me (would have liked wider toe box and had some heel slip) but worked. The Leadville’s replacement Hierro (and many other shoes, Nike Wildhorse, multiple La Sportiva models) has a debris collar that places an upper height limit on the tongue position, thus squeezing my instep. Saucany Peregrine didn’t work. Going wider only gets sloppy fit elsewhere. I have some 13 D Lone Peaks that, with strategic lacing, might work with thin socks-it fits great everywhere but instep and I know receives much love on the PCT, etc.. I’ve heard the Topo Terraventure might work, any other suggestions from the community? Thanks.

I didn’t know about the Hierro when I went looking for a Leadville replacement, and ended up buying the New Balance “910 v4” shoe. I have no idea how it compares to the Hierro, but it feels to me very similar to the Leadville and has better (grippier) tread.
For the record: I too have a high instep, but also a high(ish) arch.
Disclaimer: I use them for hiking, not running.
Salomon tends to fit wide, high-instep feet very well. I wear there shoes for hiking and everyday, can’t comment on specific running models.
I have a very high instep (for example at Girl Scout family camp, in order to wear cowboy boots for our horse session I had to go up 2 sizes to find a pair I could fit over my instep — most pull on boots don’t work for me). I hike in Topo Athletic Terraventures and Lone Peaks. I haven’t noticed the Terraventures having a lot more instep volume than the Lone Peaks but both work well for my feet. Have you tried the alternate lacing method for high insteps? On this web page it’s listed under High Arches and also High Midfoot. https://wiivv.com/blogs/keep-going/lacing-techniques-tailored-for-comfort-fit
Thanks for all rhthe suggestions. Have some Topos Terra on the way and will try lacing fix
UPDATE:
I tried a lot of trail runners. Here’s my experience, FYI, size 13 D, very high instep, but going with a dedicated W shoe is nearly always too sloppy. YMMV, I tried 13 and 14 and W in each size when an option
Don’t even try if you have a medium high – very high instep
Nike Wildhorse, NB Hiero. The scree collar upper is way too tight.
Might work for medium high instep: Terraventure, Lone Peak 4. I really, really wanted the Lone Peak to work, its a great shoe, but the upper is just too tight in height where the laces start for me, despite having a nice wide toe box.
What worked: Atra Timps, Brooks Cascadia 13 (both in size 14). The Timps are super comfortable, a lilttle looser than I am used to but not a problem to date. I’m also still in the stock insoles and thin soles, so plenty of fine tuning available if needed. I’m thinking there’s no way distance hike feet swelling will be a problem in these shoes. The Cascadia 13 has a larger toe box and better instep situation than the Cascadia 12 version, and is a very stable shoe, a bit more snug feeling than the Timps. I have both, namely for situations where the unconventional look of the Timps might be an issue, like traveling where I’m trying to get away with trail runners in non-backcountry social situations, or hikes with terrain where I might want the slightly more snug fit.
