Adam:
"I dramatically reduced the pain in my arch when I inserted a piece of rubber wedge into the arches. A PT recommended I use this arch support when I initially hurt my foot, but I haven't needed it for years. It raised the arches substantially. I'm a bit disappointed in the 1210s since I was hoping for an easy transition to trail running shoes. I think I may try Altra shoes next since they seem to be a bit more minimalist. I can walk barefoot or in soccer cleats for miles with no problem. It's something about more supportive shoes that make my feet hurt."

My plantar fasciitis required custom orthotics, but I can wear them with a variety of footwear as long as they have adequate volume(relatively high volume EEE foot, so my options are fairly limited, too).
I got those through a PT referred by my doctor. Two additional pair through my chiropractor later used the same type of mold for fitting, but were of different construction. They included a layer of exposed cork(?) and the layers began to delaminate after getting wet, so it would be prudent to get details on the materials being used beforehand if you pursue that option.
The "good ones" were two solid layers, heat molded, and have what I believe is cambrelle nylon lining on top. They have over 2000 miles on them, and get thrown in the wash with my laundry in a net bag with no ill effects.
My insurance didn't fully cover them, and I had pay ~$100, but that is probably the best money I have ever spent.
