I have no idea if it was phtoshopped or not, but as a martial artist who has studied a fair bit of grappling and “self defense” techniques along the way, the photo could be absolutly genuine, and the guy whos getting an eyeball massage could have walked away with little more than a nasty bruise.
If the finger, or in this case the thumb, is pressed into the eye socket OVER the eyeball, it can cause tremendous pain but the finger can actually go a joint or two back into the socket without serious damage. Conversely, if the finger goes UNDER the eye, the pain is compairitively slight but the damage can easily be catastrophic.
It all has to do with the structure of the eye socket.
