I'm looking for a list of custom pack makers across the internet. Mchale is the most famous, I would guess. Anyone else?
Feel free to leave a post if you make packs!

Search for Rodney (cant remember last name) who makes packs too.

ron at mountain laurel designs might make one for you.
Gossamer Gear might be able to.
probably any small cottage company would be likely to help you out.
R2 packs would be your best bet tho.

http://www.freewebs.com/litepacks/Rodney's packs are generally loosely based on the GVP G4 design. Rodney is happy to customize his packs to your specifications. Haven't interacted with him, but several people who are regularly on backpacker.com forums who have used him.

Not purely a "custom" packmaker that lets you design everything. Dan has a number of base packs which he highly customizes for people. Then again, I am not sure that his range of options is that much smaller than a pure custom makers since there are only so make ways to make a backpack.

They will work with you on the design, then make it for you.
I've worked with R2 and have nothing but praise for them. Helped me build a custom pack that none of the other packmakers I talked to would touch.
If you want truly unique and custom with the ability to design things yourself, R2 is the place to go.
If you want custom fit and the ability to choose accessories on a tried and true design that's been refined over years, then go with McHale.
